I have recompiled freeradius with your patch, and it's running now.
(There is a typo in the patch..: ht->table = NULL: )

Indeed, it looks like a file stream is closed more than once.

Please try to apply these changes to src/modules/rlm_passwd/rlm_passwd.c
then recompile FreeRADIUS and test again.
