Problem with EAP-MD5

I'm trying to authenticate my wireless users using EAP-MD5 protocol.

This is what I have on verbose radiusd -X -s

This is entry in my user file

001de03cf351  Cleartext-Password := "001de03cf351"

Can some one help me with this problem:

rlm_eap: No EAP session matching the State variable.
rlm_eap: Either EAP-request timed out OR EAP-response to an unknown
