I have tested this:

EAP ==> 1.1.7 ==> MSCHAPv2 ==> 2.0.5

fails with the same problem:

>   rlm_eap: Request not found in the list
>rlm_eap: Either EAP-request timed out OR EAP-response to an unknown
>EAP-request

EAP ==> 2.0.5 ==> MSCHAPv2 ==> 1.1.7

works. Answer: upgrade.
