HELP! accounting duplicate Start

Hi

I am running freeradius-1.0.1-1 and radiusclient
radiusclient-0.4.9.

there was a duplicate START and single stop on my
accounting directory 
( /radacct ) and on my radius.log I see this message. 

Mon Sep 12 13:45:53 2005 : Info: rlm_radutmp: Login
entry for NAS localhost port 5060 duplicate
