Detection of duplicate Acct packets

  RADIUS clients that retransmit accounting packets are largely broken.
 That is not really supposed to happen, except in certain limited cases.

  The server discards duplicates while it's processing an accounting
packet.  But once the packet has finished processing, it is *not* cached.
