  As has been said many times, you are not setting the Pool-Name
attribute.

  Which is OK, except for the fact that the debug log shows the server
isn't reading the "users" file.  And the radiusd.conf pieces you
posted don't include the "authorize" section.

  You deleted "files" from "authorize", and are still trying to use
the "users" file.

  Please don't break the server configuration.  The default
configuration is there for a purpose: it works.  It's frustrating to
see the number of people who go out of their way to break the server,
and then wonder why it doesn't work.
