  Again, the FAQ & documentation clearly describe this.  They *don't*
mention Acct-Terminate-Cause specifically, but they *do* mention what
is going on when the server isn't logging information you want.

  And again, running the server in debugging mode would make it clear
why it isn't being logged.  But you *do* have to read the output of
debugging mode.
