Radius don't write on MySQL radacct

I am afraid that you have more than one freeradius server installed. That
radiusd.conf doesn't belong to the process that produced initiall debug:

sql module is never called.

Find where is the installation which belongs to the radiusd.conf that you
have posted and start that radiusd instance.
