buffered-sql doesn't make sense to me

Not exactly. decoupled-accounting doesn't use default vitual server to
write to detail file. You define an extra virtual server that only writes
to detail file and reference it in listen section for accounting (global
one, in radiusd.conf). Use of virtual servers in listen section is
explained in raddb/sites-available/README. In that way accounting virtual
servers have nothing to do with virtual servers doing authenticatione.
Hence the name - decoupled-accounting.
