Odd authentication behavior

  Does the log contain Access-Accept?

  No.  Therefore, FreeRADIUS isn't authenticating the user.

On my Mac OS X 10.5.8 client I turned on the Wireless adapter, and selected the SSID that uses freeradius to authenticate, which prompted me for a password. I entered a bad password which gave me another password prompt. Before I could try the second time, the wireless adapter acquired an IP address and was allowed to pass traffic on the network. I gained full network connectivity while the Authentication dialog was still on screen.
