Upgrade to v2.1.9 - ntdomain logon issues

Hello,

We've recently upgraded FreeRADIUS from v1.1.8 to v2.1.9. We use it
together with Cisco AP to do WPA2 Enterprise using PEAP. All users and
machines accounts are stored in LDAP (OpenLDAP) and Samba v3.0 is
being used to provide a NT domain to Windows users. OpenLDAP contains
the ntPassword and lmPassword attributes for all users and machines.

Prior the update, everything was working fine - that is: Windows
machine authentication (after it boots), standalone logon and domain
logon.

After the update to v2.1.9, the domain login now fails constantly
(Windows machine authentication and standalone logon work as
expected).

When domain logon fails, we keep seeing in the logs "[eap] Identity
does not match User-Name, setting from EAP Identity.". We do not do
any User-Name rewrite. With tried with and without with_ntdomain_hack
in the mschap module, with and without the inner-tunnel. We also tried
to define a realm with the "nostrip" option w/o success.

Any insights would be appreciated.

Here is our FreeRADIUS configuration followed by the Accept-Request we
get which fails during domain logon :
