It's more than just that one configuration file. radiusd.conf has to be loaded. radiusd.conf then loads everything in the "modules" directory. Which then loads the radutmp file.

No. That would produce a different error message.
Radwho reads the radutmp configuration to see where the radutmp file is located. Once that's done, it reads the radutmp file.
Again, this works in the default configuration.
