[FreeRadius 2.2.8] Error: ASSERT FAILED event.c[164]: request->thread_id == NO_CHILD_THREAD

Hi,
I configured freeradius 2.2.8 with the coa update options according to the
documentation.
/etc/freeradius/sites-enabled
Auth-Type MS-CHAP {

It only works for the first request. Then I got the following error:

What i'm doing wrong? Am I missing something?
I am also not been capable of returning the custom attribute.
