In accountting with openser + radius in should insert only one insert for every call made .
FreeRADIUS with SQL accounting will insert a row for each session when accounting starts, then update that row when accounting stops. I'd guess that the session with an AcctStopTime of all zeros that you posted hasn't actually stopped yet.
