Search for my previous post listed by the subject "Authenticate/Attributes based on NAS-IP-Address". I did this same thing - it requires modifying the SQL SELECT statements in sql.conf, and in my case, adding several columns to the radgroupcheck, radreply, and radgroupreply tables (one fo reach NAS/Client).
