Well that looks like the user is connecting and disconnecting several times, looking at "Connect" and "Disconnect".

Sure about that? FreeRADIUS doesn't just randomly write log lines when it gets bored... I would start freeradius, e.g. radiusd -X > /tmp/log connect just that device and watch the logs for this to occur, then kill off freeradius and look at the full debug output. That will tell you what is actually happening. If you're still not sure, send the full debug output here.

Run in debug mode, look at the previous Access-Accept for that session. The NAS should send the User-Name from the Access-Accept in the accounting packet. If it doesn't, there's not a lot you can do.
