  Is the LDAP module (or instances of the LDAP module) listed in the
"authenticate" section?

  What is the debug output for one PAP request?  See CVS head,
raddb/sites-enabled/inner-tunnel for a sample configuration that lets
you debug the inner tunnel policy without doing EAP in the outer tunnel.
