You're decoding password-with-header, but your password has no header. Your other password encoding is MD5, which is incompatible with mschapv2.

In 3.x, the correct header is {clear}. I don't know if this is the case for 2.1.12.  You could dig into the old code/documentation to find out... but you really should just upgrade.
