OK, what have you set for the "driver" option in mods-enabled/sql?

It should be "rlm_sql_sqlite".

If you set it to "rlm_sqlite" then you'll get the error you are
getting.

If that doesn't help, to save more guesswork please post the full
output of radiusd -X.
