Ops, I forgot to mention that you need to remove the “#” from “column_name”, “packet_xlat” and “reply_xlat” e.g: https://github.com/FreeRADIUS/freeradius-server/blob/v3.0.x/raddb/mods-config/sql/main/mysql/queries.conf#L100
