That debug output doesn't correspond with the updated config you pasted.

I suspect FreeRADIUS isn't reading the file you edited, maybe because 
it's using a different config location. Look at the top of the debug 
output and check that the file you are editing is actually being 
included. If in doubt, just to prove it's reading the correct file you 
could e.g. add some junk to the top of the file and FreeRADIUS should 
fail to start.
