Have you run the server in debug mode as suggested in the FAQ, README, "man" page, etc..?
users file has the following:
DEFAULT Service-Type == NAS-Prompt-User Service-Type := NAS-Prompt-User, Cisco-AVPair += "shell:priv-lvl=15"
