You're managing to send the VLAN twice. Don't do that. If that doesn't fix it, I suggest you consult the documentation for your NAS - those attributes are standard and should work, but your switch may require custom attributes. We assign vlans to 3Com 4400s (different product of course) and those attributes work fine, so I suspect you sending it twice is confusing it.
